General Position Description

Under general supervision of the Nurse Manager, the Health Education Coordinator will be responsible for enrolling patients for and facilitating health education programs, working with community partners to promote healthy lifestyles and coordinate health education efforts in the community, and developing high quality health education and outreach programs.

Duties and Responsibilities

• Define specific educational objectives for program
• Plan and implement educational and outreach programs, developing criteria, staffing and material requirements to reach program objectives
• Oversee posting, dissemination, organization and inventory of all health education information
• Increase awareness and understanding of health education programs, services and activities among CommunityHealth’s providers and patients
• Recruit, train and oversee health education volunteers
• Track program outcomes and analyze data
• Participate in public education and information efforts to increase awareness and understanding of health programs, services, and activities in the community.

Qualifications

• Bilingual in English and Spanish, both oral and written, is required.
• Bachelor’s degree or higher in Public Health, Health Education or related field. Three years work experience required.
• Experience in program implementation and oversight, documentation, and data collection
• Strong analytical and problem- solving skills with a sharp attention to detail and consistent, reliable follow-through
• High initiative and flexibility to effectively multi-task and work within a dynamic work environment
• Experience working with low income and undocumented population. Experience in mental health field a plus
• Excellent oral and written communication skills
