CommunityHealth selected for $100,000 award to become Patient-Centered Medical Home

As part of our ongoing focus on improving the quality of patient care for the uninsured, CommunityHealth is proud to announce that we have been selected as one of two free clinics nationwide to participate in the first ever

Americares/BD Advancing Community Health award program! This was a highly competitive process, and selection entitles CommunityHealth to a $100,000 award and technical assistance in support of our work to attainaccreditation from the National Committee for Quality Assurance, officially recognizing CommunityHealth as a Patient-Centered Medical Home.


To qualify as a patient-centered medical home, clinics must actively engage patients in their care plans by connecting them to medical and social resources in their communities, offering easy access to care according to patient preferences, and providing a comprehensive approach that addresses primary care and mental health needs. Becoming a patient-centered medical home also increases communication at the clinic level and allows providers to offer coordinated care to patients including prevention, wellness, acute, and chronic care.
