Ways to Give

There are many ways you can contribute to CommunityHealth to help provide quality healthcare to the uninsured in our community. Donations covered operating expenses incurred in providing over 25,000 medical visits and filling over 56,000 prescriptions for our patients in 2010.

Donate by Mail or Phone

You can send your donation to CommunityHealth, 2611 W. Chicago Avenue, Chicago, IL 60622. Please call Judith Blank,  Director of Development, at (773) 969-5942 to make a donation through your Visa or MasterCard.

Donate Online Now

You can donate online by clicking the Donate Now button on any page of our website and using your Visa or MasterCard card. This is a secure and easy way to support the Health Center. You can also make a gift in honor of or in memory of a loved one through this site.

In honor of…

Make a donation to commemorate a holiday, birthday, wedding, or other special occasion. The special person you honor will receive a letter from CommunityHealth notifying him or her of your thoughtfulness and generosity.

In lieu of flowers…

When a loved one has died you may chose to have family and friends make a donation to CommunityHealth rather than sending flowers. CommunityHealth will send a personal thank you to each donor and send the designated family a list of donors and their addresses (gift amount will be keep confidential).

Matching Gifts

Many companies offer matching gift programs to encourage employees to contribute to charitable organizations. Most of these programs match contributions dollar for dollar, and some will even triple the amount of your gift! Please check with your employer about their matching program, complete the required forms, and send to CommunityHealth.

Challenge Grants

An anonymous donor has agreed to match new and increased donations dollar-for-dollar up to $50,000 automatically doubling the impact of your gift. New funds include funds pledged and collected either from a new donor to the organization or from a prior donor, but only to the extent of the increase in that donor’s gift to the organization over that donor’s gift in the prior year. The amount that the Foundation ultimately awards to CommunityHealth will depend on subsequent fundraising by the organization.

Gifts of Securities

A gift of appreciated securities held for more than one year can provide significant benefits to you as well as CommunityHealth. If you are interested in donating securities, please call Judith Blank at (773) 969-5942 for instructions and form.

Supporting the Gala

Planning for our 2012 Gala is underway. Ten percent (10%) of our overall operating budget is generated through this event. If you are interested in learning about opportunities to support this event, please contact Madalyn Messer-Brooks at 773-969-5918 or mmesser@communityhealth.org .

Naming Opportunities

CommunityHealth will create individualized recognition plans for donors interested in a naming opportunity, particularly those wishing to honor or memorialize individuals through their gift. A gift may be given as a one-time contribution, or pledged over a two to five year period, depending on the gift amount.

Planned Giving

Planned giving integrates a donor’s charitable gift with his or her overall financial, tax, and estate planning goals to maximize benefits to both the donor and CommunityHealth. More information coming…
