Becoming a CommunityHealth PatientCommunityHealth is committed to providing free, quality health care to our patients. All patients are seen by appointment only for all (primary & specialty care and case management) services. SAME DAY APPOINTMENTS ARE NOT AVAILABLE. The health center serves anyone who does NOT have any form of health insurance (including Medicare, Medicaid, KidCare or any medical card) and is within income guidelines, below 250% of the Federal Poverty Level (FPL). The only exception is for people who have Medicare Part A. Medicare card must be provided. 2008 Federal Poverty Guidelines
